Who will look after me?

When your registration is processed and accepted you will be allocated a named GP. You will be informed of this by letter or email.

Accountable/Named GP for all Patients

All patients registered at Poplar Grove Practice will have a named accountable GP. This GP will oversee your overall care but may not be the doctor you see every time you visit the surgery for an appointment. If you prefer to see a specific doctor, you can request this via our EConsult form or via our Patient Services Team. All new patients registering with us will be notified of their named GP via email or letter.

Changing your Named GP

We need to ensure we always maintain safe and appropriate GP list sizes for our clinical teams.  If a patient requests to see a particular GP, reasonable efforts will be made to accommodate their preference, but recognising there are occasions when the practice may not feel the patient’s preference is suitable

If, during the time you are registered at our surgery, you wish to change who your named GP is, we will ask you to submit an official complaint, in writing via email address feedback.poplargrove@nhs.net , stating the reason for making the request to change GP.  As per our complaints policy, we will initiate an investigation into the concerns raised and inform you of the outcome within 20 working days.
